Everyone will be required to have an initialDUI Screening: I will evaluate the problem, a short social and substance abuse history, You will be required to take two tests for the evaluation. Bring me your record of your blood alcohol content, a court document that lists the charges and TR/CR number, and most important is the CITATION DATE The courts wll also give you a list of other agencies address and phone numbers to call. Education- Level II. Will be provided each Saturday (Part I and the following Saturday Part 2). The Parts don't need to follow each other.This is the minimum, but you may need additional hours if so required, and will be noted in the Screening Results.The cost for this service is $80 for each Part to be paid at the beginning of the session.
Treatment Group Counseling is provided each Monday, Tuesday, Wednesday and Thursday from 6:00 PM to 8.00 PM. at the cost of $20.00 per 2.0 hour session.Here we develope a comradeship of thougts, ideas,and therapeutic techniques such as Cognitive Therapy and Motivational Interviewing which have been designated as "Best Practice" techniques to speed the process of reviewing what, and how you made the decision to drink and drive. There are fun activities too. Everyone shares their ideas during the counseling sessions. The focus of attention is what you need to do to prevent another DUI.
Evaluation requirements: One can also be make an appointment on Friday from 10:00 AM to 4:00 PM, You may need to have an interlocking device installed when the MVD tells you to do so. . Do not get this device until the MVD sends you a letter telling you to have it installed or present yourself at any MVD Office. I will suggest an agency if you are required to have it installed. You will need the electronic submission form, and then seek an agency for he installation of the interlocking device prior to getting the device. When you visit the MVD have a documnet avaiable (SR22) if required. Contact them or they may have already told you in one of their letters you will surely get. This can be obtained individually. Look on the MVD wall for an independant SR22 provider. Also, you can contact your insurance provider and they will assist you.Call for additional information.You must have in your possession a MVD computerized form that tells the MVD exactly was provided to you. I am licensed to prepare it for you..The name of this document is an Alcohol/Screening Report, and Education and Treatment Report which must be provided to MVD when you are eligible to reinstate your license, and have the interlock installed.
For those that visit me for out of state requirements, I must follow their requirements and then they will send you a clearance form to have your Arizona license/other reinstated.
If your driver’s licensed has been revoked, I will complete the packet $70.00 rather the full price of $250.00 if you completed a 36 hour program at Alchemy DUI Services. The packet must be competed in its entire form without white out, or strike through, and no errors. Bring a copy of any education certificate, If you have completed a program elsewhere, that's OK. the cost of the MVD Packet is $75.00 Part B will be completed by the court of your record.. I will mail your packet the same day.
